Bile Flasks will not heal you, refilling your Fervor instead. You look at the regenerating fervor and think: "ah, I get it, this is the Wizard build!" Fortunately, the changes to healing make vampirism strats (ie, Saeta Dolorosa and the Molten Heart of Boiling Blood) extremely effective, with an initial strike or the first kill on a map segment often instantly restoring a health orb. In the early game you won't have the Fervor required to really spam the Tiento, so stick to the vampirism route. Yep, whole new free DLC with a New Game+ including new batch of bosses, penances and various quality of life changes. Requires Weight of True Guilt to enter after destroying the statues. New animated cutscenes are attached to them. IMO that doesn't have to be this way. The reward for completing this Penance is the relatively inconsequential Reliquary of the Suffering Heart prayer bead, which grants you gradual health regeneration in addition to the normal instant health restoration you get when using a Bile Flask. It's the bosses, as usual, that are the real problem. I also found that my go-to Guilty Heart Tiento cheese strat was dramatically less effective than on the other Penance runs; that strat relies on heavily on your sword to do the damage (theoretically Ferverous Blood would work just fine, but in practice I found this difficult as I mentioned above), while your fervor is reserved purely for defense. Nothing can ease your pain. ALL RIGHTS RESERVED. Notes: You need to have the Bead: Weight of True Guilt equipped to enter the challenge rooms. You also need to kill 2. I have to say "I suspect" here because I basically "cheated" on this Penance (hence the asterisks above): I did an end-run around the Bile Flask effect by using the Reliquary of the Sorrowful Heart I obtained from my True Guilt run to convert my useless Bile Flasks into highly valuable Fervor Flasks. You will also get The Path of the Unworthy achievement and the true Apostasy Skin as rewards. THE COVID-19 VACCINE IS KILLING PEOPLE ON A LARGE SCALE. I wonder how is it even possible to beat the final boss with that penitence. Minor: Enemies respawn when you leave and then return to an area. So, not only could I use the same Tiento cheese strat I utilized in the previous run, but that strat was actually made more effective by the fact that during the brief periods I was exposed to damage, I couldn't really lose more than one or two life orbs. Normally, whenever you die on this penitence, you leave all your tears of atonement as a Guilt Fragment at the place you died at (Dark Souls style), but if you die with the Immaculate Bead equipped, said Guilt Fragment doesn't appear, thus ripping you off every tear you had, which is unfair. It's much more valuable to have lots of flasks to activate your lightning attacks, and because of your fervor regen (and the fact that you can ignore defense buffs and debuffs, giving you more bead flexibility and making the +damage sword heart have no downside) you heal yourself much more efficiently by going purely on offense with the lifestealing Prayer. Penances are more like double-edged swords that make certain aspects of the game harder while making other aspects easier. I'll highlight which tools I think are preferable for each specific Penance in the discussion below, though certain items - especially traversal tools (like the Linen of Golden thread relic), which will allow you to grab Empty Bile Flasks, Health and Fervor upgrades earlier then you would otherwise be able to - are highly recommended for any NG+ run.
